Why is my relationship "On hold"?

"On hold" is what the app calls a Suspended relationship. Nothing about the underlying relationship is deleted: tasks, history, and settings are all still there, but day-to-day features (task assignment, chastity locks, active consequences) don't function while it's in that state.

How it gets there and back

A Sub requests release from an Active relationship, which moves it to Suspended, on hold, awaiting the Dom. From there, a Dom either approves the release, moving it to Ended, a separate and final state, or brings it back to Active once things are sorted out. A Dom can also revoke a relationship directly, straight from Active to Ended, without a Sub's request first.

What changes while it's on hold

New tasks can't be assigned. Chastity timers that are already locked keep running (going on hold doesn't remotely unlock anyone), and the relationship stops counting toward Home's digest or contributing to stats until it's back to Active.

If you want to end things rather than pause them, that's Ended, reached either way above, and unlike on hold, it's treated as final.
